If you wish to use a photograph that was taken by a professional photographer and they retain the rights to the photograph, please have them complete and sign the below photo release form.  Please note, some photographers / Photo Studio will provide you with their own photo release free of charge.  The below release is being provided in the event one is not provided by your photographer.
